Man dies following serious injuries; police fail to complete wellness check

A man has died of “serious injuries” after a report was made to Maidstone RCMP in the early evening of April. 3 at a home in the RM of Britannia.

His family has been notified.

Police initially received a wellness check request earlier that morning but according to the release, a physical check was not done.

As a result, the Saskatchewan Serious Incident Response Team (SIRT) has been informed and they will investigate to see if there were any breaches in policy regarding the initial check.

Meanwhile, Saskatchewan RCMP will do a separate investigation and the Major Crimes Unit with the help of the North Battleford Forensic Identification Services will investigate the situation surrounding the death alongside Saskatchewan Coroners Service.

Friday afternoon, the Ministry of Justice released further information on the incident, confirming the age of the deceased to be 68 and that the wellness check had initially been requested by a relative.

SIRT’s investigation will examine the conduct of police during this incident, including the actions and omissions associated with the police response to the initial call for service. The RCMP will maintain conduct of the investigation into the circumstances of the man’s death.

A final report will be issued to the public within 90 days of the investigation ending.
